Controlled Articular Rotations (CARs)

This exercise improves active range of motion, movement control, and overall joint health through slow, controlled movements.

What is the purpose of this exercise?

  • A neck CAR is performed by slowly and with control moving the head through its largest pain-free circle, keeping the rest of the body still

  • CARs are designed to take each joint through its full, active range of motion to help maintain the health of the joint capsule and surrounding tissues
  • The idea is to inform the patient about his available active range of motion, identify his weak or painful spots, improve his neck mobility and control, and prevent stiffness and injury
  • Many think of CARs like “joint hygiene”, daily brushing for your joints to maintain long-term function, use your range of motion, or lose it

Who should do this exercise?

Everyone, especially patients experiencing a limited or painful range of motion

 

How do I scale the difficulty?

For an easier version: Reduce the range by performing a smaller, pain-free arc instead of a full circle. The patient can do the exercise lying on his back (supine) to minimize postural demands. 

For a harder version: Have the patient change positions. Move from sitting → standing → tandem stance → single-leg stance, for example.
