How often should I reassess my patient?

Clinician's discretion; every ~4 weeks or 3-6 sessions

When determining when and how frequently to reassess your patient, it is up to your clinical discretion. The following may be considered:

  • The patient-report
    • If the patient reports improved (or reduced) ability to participate in activities of daily living
    • If patient symptoms are improving (or worsening)
  • Your targeted plan of care
    • What will your frequency of services be? How often will you have the opportunity to reassess?
  • Payors and Providers
    • If your patient requires additional justification for insurance authorization 
    • If your patient is scheduled for a follow-up with the referring provider 

Generally, a reassessment is recommended every ~ 4 weeks or between 3-6 treatment sessions.
